Chanmi Park, MD, MPH

Dr. Park is a geriatrician and clinical investigator with clinical and research expertise in geriatric pharmacoepidemiology and health services research. Her goal is to improve the transition of care process by adapting innovative care interventions and safe medicine use for older adults with frailty. 

Dr. Park has conducted a prospective cohort study enrolling 201 older patients to examine the clinical and functional outcomes of older adults hospitalized with pneumonia. She found that functional decline was common, regardless of the pneumonia severity, particularly in those with frailty.

To better understand the issues around the care transition period, Dr. Park conducted a qualitative study of health care professionals to investigate barriers in transitional care management from a hospital to the community after acute illness. The study provided an in-depth understanding of the unmet discharge planning needs for older adults.

Dr. Park’s research interests also include geriatric pharmacoepidemiology and the safety of medications in acute and post-acute care settings. She has used a national inpatient database to study the in-hospital adverse events associated with gabapentin use after major surgery.

Currently, she is analyzing the outcomes of anticoagulant and statin intensity in older adults with frailty using Medicare data.
